Year-end local church statistics available online (Table 1, Table 2, Table 3) –  Complete online annually.  CLICK HERE to login to complete your church statistics report. Must be completed by 4:00pm, Friday, Feb. 11, 2011.

14th 2010 Apportionments: Friday, January 14, 2011 is the last day for receiving 2010 apportionments. This is not a postmark date, but the date by which monies must be received in the conference office to be included in 2010 receivables. Monies received after this date will be credited to 2011. Include a completed apportionment giving form with your payment. Always include your church’s GCFA number on all forms and checks. When paying in January, mark clearly if the payment is for 2010 or 2011 apportionments.

Year-end local church statistics (Table 1, Table 2, Table 3) close on Feb. 11th at 4:00 p.m.  Complete online annually. This form is available online January 6, 2011.  Must be completed by February 11th at 4:00 p.m.

Workers Comp Audit.  Complete online annually.  Report actual payroll for 2010.  Click here  for the 2010 Workers Compensation payroll survey.  Must be completed by February 18th.

Deadline for completing church and pastor profiles. These forms should be reviewed and updated annually by all church SPR Committees and all clergy in the Florida Conference. Pastors having January/February consultations with their District Superintendent should insure that their church and pastor profiles are completed one week prior to their scheduled consultation. All other clergy should review and update these forms annually during the time frame allowed, Dec. 1, 2010 to Feb. 18, 2011.  

Though you may not anticipate a move, it is important that both the Pastor Profile and Church Profile be current and up to date at the appointment-making session of the cabinet. The reports should be an honest reflection of the pastoral ministry and the profile of the church.  

Church Profile - should be reviewed and updated annually. 
All church SPR Committees must complete online annually and may begin work on this in December. The Church Profile deadline requires it to be completed online no later than one week prior to the pastor’s DS Consultation if they are having one. The final date to update the Church Profile is Feb. 18, 2011.

Pastor Preference for Appointment and Staff Parish Advisory Recommendation. 
Only Pastors and SPRCs making an advisory request to the Bishop and the Cabinet for a move in 2011 will complete the Preference for Appointment or SPRC Advisory Recommendation forms this year. If requesting a move, contact your District Superintendent to schedule a consultation appointment and to request the appropriate Advisory Recommendation form.  (Please refer to a copy of the  revised Cabinet Appointment Making Guidelines 2010 for more detailed information.)

Child/Youth Protection Policy adoption – Review & renew annually.  Mail a signed copy of the Annual Review & Renewal Signature page of the Child/Youth Protection Policy to the district office by March 31st. (Should the policy undergo major revisions, mail a complete copy to the district office).
NOTE: The Child/Youth Protection Policy Template was revised in December 2009. Changes include the addition of information related to online safety, clergy confidentiality, and reporting requirements as well as other revisions.  We recommend you review the entire policy template.

 

31st Fiscal Responsibility and Management Policy: Financial Controls, Policies, and Procedures - Review & renew annually. Once in compliance,  mail a signed copy of the one page Annual Review and Renewal Document to the district office by March 31st.     
31st Sexual Harassment Policy adoption– Review & renew annually. Mail a signed copy of the Annual Review/ Endorsement Renewal to the District Office by March 31st. (Should the policy undergo major revisions, mail a complete and signed copy to the district office).

15-31 Annual Ministry Protection Property and Casualty Insurance Review (Annual Survey). Review insurance coverages and update online insurance database annually. Annual survey period is August 15-31. This database is available online year round to enable churches to update insurance data as changes occur. Click here for link to the insurance database on the Ministry Protection website.

Annual Workers Compensation Salary Survey. Complete online annually. Annual survey period is August 15-31. This data is collected as part of the annual Ministry Protection insurance survey. Please be sure to project your payroll by category through the end of 2011 when completing this section.

Annual Charge Conference forms available online Thursday, Aug. 25. These forms are to be completed annually by each local church in accordance with the district superintendent’s annual charge conference directions.

16th Health Insurance Adoption Agreement - All churches must return the adoption agreement found at the bottom of the document either agreeing to participate or waiving participation in the Health Benefits Plan for Lay Employees in 2012. Eligible Clergy are required to participate. Click here for more information.
     
31st Clergy Assessment Process “Understanding our Ministry Together.” The clergy assessment process for 2011 is available online at www.flumc.org under the Ministries Tab: Center for Clergy Excellence. Clergy and the Staff/Pastor Parish Relations Committees should meet in the fall to complete this process. The report is due to the District Office every year by Oct. 31. Click here for more information.

30th Local Church Leaders data entry (incoming local church lay officers contact information for the year 2012) should be completed within two (2) weeks of your Annual Charge/Church Conference in the fall. Current leaders and contact information can be accessed on October 1, 2011 via the church login on the Florida Conference website, www.flumc2.org. Updates and changes in local church officers should be reported to your District Offices as they occur throughout the year.
