2005 Calendar of Forms and Reports
Deadline Dates For the Local Church

    January
? 1st The church & pastor profiles should be reviewed and updated annually by
    all churches and all clergy in the Florida Conference.
Pastors having January consultations with their D.S. should insure that their church profiles, pastor profiles, and move preference forms are completed one week prior to their scheduled consultation. All other clergy should review and update these forms annually during the time frame allowed, November to January 31st.
     
?   Church Profile - should be reviewed and updated annually.
   

All Churches must complete on-line annually and may begin work on this in November. The Church Profile deadline requires it to be completed on-line no later than one week prior to the DS Consultation. The final date to update the Church Profile is January 31st.

     
?   Pastor Profile – should be reviewed and updated annually.
   

All Pastors must complete on-line annually and may begin work on this in November. The Pastor Profile deadline requires it to be completed on-line no later than one week prior to the DS Consultation. The final date to update the Pastor Profile is January 31st.

Staff-Parish Preference Form – The 2005 SPRC is to complete the form on-line and mail a signed hard copy to the district office by the date of the pastor’s consultation.

   

This form will be available on-line January 1st and must be completed one week prior to the pastor’s consultation with the D.S. (It is the pastor’s responsibility to notify SPR Chair of the date of the Clergy Consultation to insure that this report is received by the DS on time.)

Pastor Preference Form – All Active Clergy are to complete the form on-line and mail asigned hard copy to the district office by the date of the pastor’s consultation.

    This form will be available on-line in November and must be completed one week prior to the pastor’s consultation with the D.S.
     
? 10th 2004 Apportionments: Monday, January 10, 2005
   

is the last day for receiving 2004 apportionments. This is not a postmark date, but rather is the date by which monies must be received in the conference office to be included in 2004. Monies received after that date will be credited to 2005. Include a completed apportionment giving form with your payment. Always include your church’s GCFA number on all forms and checks. When paying in January, mark clearly if the payment is for 2004 or 2005 apportionments.

     
? 31st Annual Workers Compensation Salary Survey
   

This form will be available on-line January 15th  and must be completed by January 31st

? 10th Year-End Local Church Statistics (Table 1, Table 2, Table 3)
   

This form will be available on-line January 1st and must be completed by February 10th.

     
    March
? 31st Child/Youth Protection Policy adoption Review & renew annually.
   

Mail a signed copy of the Annual Review/Endorsement Renewal to the District Office by March 31st. (Should the policy undergo major revisions, mail a complete copy to the District Office).

1st FLCIS data entry form (incoming local church lay officers for the next year) – should have been completed with the Annual Charge Conference Paperwork. Final copy should be sent to the District Office no later than December 1st.
